چکیده

In recent years there has been a great deal of interest in the so-called carcinoid syndrome. This syndrome has been observed in many cases of carcinoid tumour of the gastro-intestinal tract with extensive metastasis. It consists of certain transient and permanent vaso-motor changes, bronchoconstriction, hyperperistalsis, oedema and ascites, pellagra, and mural and valvular endocardial sclerosis with symptoms and signs of pulmonary and/or tricuspid heart disease. In most reported cases, only some of these components have been evident. It is generally believed that most of the signs and lesions are caused by 5-hydroxy-tryptamine released from the tumour tissue, but the exact mechanisms behind the changes are insufficiently known. The development of endocardial sclerosis in the right side of the heart is especially difficult to explain: though several explanations have been offered, no valid proof has been published in support of any of them. All recent authors agree that the endocardial lesions are acquired late in the course of the tumour disease. However, only a few of the reported patients have been studied before and after the development of their valvular lesions and for this reason the authors believe the present case to be of interest. Our studies include right-heart catheterization during a spontaneous flush episode and verification of the lesions by autopsy. The early history of this patient and some of the haemodynamic studies have been published (Thorson et al., 1954; Waldenstrom and Ljungberg, 1955; and Thorson, 1956).

Occurrence of left-sided heart valve involvement before right-sided heart valve involvement in carcinoid heart disease.

Carcinoids are rare neuroendocrine tumours that occur primarily in the gastrointestinal tract. Carcinoid heart disease is characterized by fibrous plaque deposition on the endocardial surface of the cardiac valves and chambers. It affects the right heart valves in 85% of cases and the left heart valves in 15%. We present an unusual case of a patient with metastatic carcinoid heart disease in wh...

Carcinoid Heart Disease and Review of Literature

Carcinoid Heart Disease (CHD) is one of the rare causes of acquired valvular heart disease. CHD develops as a systemic manifestation of metastatic neuroendocrine tumors (NETs). When these tumors metastasize to the liver, they can release vasoactive amines into the systemic circulation, which can result in carcinoid syndrome and CHD. The key for diagnosing CHD is a thorough history and physical ...

¹³¹I-MIBG scintigraphy in Carcinoid tumor with liver metastases (Case report) [Persian]

A 65 years old male presented with abdominal pain (R.U.Q) and constipation, associated with weight loss and anorexia since 40 days prior to admission. Serum biochemical tests were normal. Colonoscopy showed hemorrhagic polypoid lesion in the cecum. Barium enema revealed filling defect in the cecum. Sonography revealed a target-shaped lesion (3cm×2.5cm) in upper segment of the right hepati...

GASTROENTEROPANCREATIC NEUROENDOCRINE TUMORS (GEP-NETs) - APPROACH TO DIAGNOSIS AND MANAGEMENT

Clinical Presentation and Natural History The clinical course of patients with GEP-NETs is highly variable. Some patients with indolent tumors remain symptom free for year even without treatment. Most patients with non-functioning tumors due to lack of symptoms related to hormonal hypersecretion are diagnosed late in the course of the disease. Clinical signs and symptoms are due to tumor mass w...

Metastatic carcinoid tumor to the heart: echocardiographic-pathologic study of 11 patients.

OBJECTIVE We sought to investigate the clinical and echocardiographic (echo) characteristics of metastatic carcinoid tumor in the heart. BACKGROUND Right-sided valvular dysfunction is the hallmark of carcinoid heart disease. Cardiac metastases are uncommon in carcinoid syndrome. Features of patients with metastatic carcinoid tumor involving the heart (MCH) have not been well described. METH...
